    Transition to adulthood

    Moving from children's health services to adult services is transitioning.

    As you transition to adult services, you and everyone involved in your care will have time to discuss your healthcare needs.

    We understand it's difficult to leave a team of doctors and nurses you've known for years. But by learning and taking part in the transition process, we hope you'll feel more confident.
