Blue Badge application support
Your Blue Badge application must include:
- a passport style photo
- proof of identity
- proof of address (this cannot be the same as your proof of identity)
- proof of eligibility (if you automatically qualify)
- proof of diagnosed conditions (if you do not automatically qualify)
- £10 administration fee.

Send us additional documents
Sometimes, after you apply for a Blue Badge, we ask you to send additional supporting documents.
