Enablement at home

Our enablement support services are designed to help you  develop or regain the skills needed to manage daily tasks supporting you to live your best life. Meaningful activities can lead to improved:

  • fitness
  • better mental health
  • greater sense of purpose.

Enablement aims to support you to feel safe at home and in your community.

We offer two types of enablement support: at-home support available from our Kent Enablement at Home (KEaH) Team or specialist enablement support from our Kent Enablement Support (KES) Team.

At home support (KEaH)

Our Kent Enablement at Home (KEaH) will work with you to set personalised enablement goals, for example:

  • improving your confidence after a fall
  • giving you guidance in preparing meals following a change in circumstances
  • reconnecting you with your social activities
  • identifying equipment that could help you with independent living
  • helping you to return or remain at home and feel safe and happy.

Specialist enablement support (KES)

Kent Enablement Service (KES) offer a short-term service designed to help you achieve your personal goals and enhance your independence at home and in the community.

Our support is available for older people who have social care needs and individuals over 18 living with:

  • learning disabilities
  • autism
  • mental health problems
  • physical disabilities.

Referrals to KEaH

A referral to KEaH can come to us via:

  • a stay in an acute or community hospital
  • your GP
  • a community nursing staff
  • the ambulance service
  • a family member, friend, neighbour or your community
  • day centre.

When we receive your referral, one of our Enablement Supervisors will assess it. Where they will talk to you to help you set personal goals to keep as much independence as you can.

Referrals to KES

Referrals to KES are made through social services.

To decide what support is best for you we start with a lifestyle questionnaire to understand your needs and goals. We develop an enablement plan with your objectives. Our trained staff will work with you to achieve your goals and we will regularly review your progress and adjust the plan as needed.

Once we have learnt what support is best for you, we can offer you help with:

  • daily living support (cooking, cleaning, budgeting, shopping)
  • employment help (accessing to employment and volunteering)
  • being involved in your community
  • accessing public transport
  • technology support (accessing devices to help your independence, visual aids and teaching new digital skills)
  • your confidence in general.

How long is the support for?

KEaH

The support you will get will be in your home. We’re here for you as long as you need. The time we spend together can vary for each person.

Usually, you will stay with us for support for around two weeks. This can vary due to the support you need. When you're happy and comfortable, and you feel you've met your goals, you can choose to end your support with us. Many people leave our service without extra support.

KES

This depends on your enablement pathway. There are 4 pathways:

  1. Self-directed support which includes signposting to our help desk for information and advice on support services, charities, and local groups.
  2. Community support for up to 8 weeks and reviewed at 4 weeks.
  3. Intensive intervention support for up to 4 weeks and reviewed at 2 weeks.
  4. Hospital Discharge Service covering 3 mental health hospitals, supporting 2 weeks in the hospital setting and 2 weeks in the community.

Cost

Our enablement services are free at the point of referral. If you need help after our support ends, we can talk about your options, including costs.

Help once you meet your goals

If you feel that you still need extra support, that's okay. We will carry out a review and will talk with you about the ongoing support you need. It may be that you need long-term care and support from a domiciliary agency. This is an agency we commission for your care and support.

Our Financial Team will check if you can help pay towards your care and support. This could be while you wait for your new domiciliary agency to start, and while you're still receiving help from us. During this stage, our services will be chargeable.
